During this 60–75 minute visit, we’ll take a comprehensive look at your concerns, identify the root cause of your symptoms, and create a personalized treatment plan designed to help you return to the activities you love with confidence.

Your initial evaluation is designed to uncover the root cause of your symptoms. I’ll perform a comprehensive assessment that may include evaluating your posture, movement patterns, strength, mobility, breathing mechanics, core function, and pelvic floor muscles. Depending on your symptoms and goals, this may include an external and/or internal pelvic floor examination. Every step will be explained beforehand so you understand what I’m assessing and why, and nothing is ever performed without your informed consent. Insurance Information
Momumental Physical Therapy & Performance is a cash-based practice, allowing me to provide the personalized, one-on-one care you deserve without the restrictions of traditional insurance.
All of my services are covered under your out-of-network benefits.
I am in-network with Medicare and I’m also a TRICARE-certified non-network provider, allowing me to treat patients with TRICARE Select and TRICARE Reserve Select plans. If you have questions about your coverage or benefits, I’d be happy to discuss your options.

Strong Start Package
Includes an initial evaluation and 4 follow-up sessions.
Momumental Healing Package
Includes an initial evaluation and 8 follow-up sessions.

Includes an initial evaluation and 12 follow-up sessions.
Birth Prep Package
Includes 4 sessions focused on pelvic floor muscle relaxation, perineal massage, labor prep exercises, breathing techniques, pain management and positioning during labor and pushing, and partner education.

Getting started is easy! Simply fill out the contact form by clicking the “Let’s Get Started!” button at the bottom of this page. I’ll personally reach out to you within 24 hours to learn more about your fitness background, current activity level, goals, scheduling needs, and any equipment you have available. This allows me to create a personalized training plan so we can make the most of your first session together.

Your first in-person session is all about getting started — not wasting time on paperwork. I’ll take a few quick baseline measurements, like weight and body composition (only if you want to!). Then we’ll dive straight into a personalized workout designed for your goals, fitness level, and comfort.

Training sessions are primarily held in my private studio, located in Cumming. The studio is fully equipped to support a wide variety of strength and conditioning workouts in a private, one-on-one setting. In-home training may be available based on your location and my availability.

This isn’t your average fitness program — it’s expert-guided training designed specifically for women. I bring the clinical knowledge of a physical therapist, the performance expertise of a strength and conditioning coach, as well as advanced training in perinatal fitness.
I offer both 40-minute and 60-minute sessions. You’ll have the option to choose 2, 3, or 4 sessions per week.

Physical therapy focuses on addressing pain, injuries, and movement limitations. It often includes assessment, hands-on treatment, and corrective exercise prescribed by a licensed physical therapist to help you heal and return to your activities safely.
Personal training is for when you’re healthy enough to exercise but want expert guidance to get stronger, improve performance, or reach specific goals. At Momumental, your training is backed by a deep understanding of anatomy and biomechanics to keep you moving well and prevent setbacks.

An internal pelvic floor examination is never required. While an internal assessment can provide valuable information and may be recommended during your initial evaluation, it is always your choice whether to proceed. Before any internal examination, I’ll explain why I believe it may be beneficial, what to expect, and answer any questions you have.
Your comfort, privacy, and trust are my top priorities. An internal examination will only be performed with your informed consent, and you may decline or stop the exam at any time. Nothing is ever done without your permission.

An internal pelvic floor examination is never required, but it can provide valuable information to help guide your treatment. If an internal assessment is recommended, I’ll explain why it may be beneficial and answer any questions before we begin. It is only performed with your informed consent.
The examination involves the gentle insertion of one gloved finger with a water-based lubricant into the vagina to assess the pelvic floor muscles. During the exam, I evaluate:
Pelvic floor muscle strength, endurance, coordination, and function during movement
Your ability to contract, relax, and lengthen the pelvic floor muscles
Muscle tone and tissue mobility
Areas of pain, tenderness, or trigger points
Scar tissue from childbirth, surgery, or injury
Signs of pelvic organ prolapse and the level of support of the pelvic organs (while physical therapists do not diagnose prolapse, we can assess the amount of support present and determine whether further medical evaluation is appropriate)
Your comfort, privacy, and trust are my top priorities. You are in control throughout the entire examination and may choose to decline or stop the assessment at any time. Nothing is ever done without your permission.

In Georgia, you can start physical therapy without a referral for the first 8 visits or 21 days (whichever comes first). If you need care beyond that, I will coordinate with your doctor to make the process seamless.
If we decide you are a candidate for dry needling, a physician referral will be needed prior to undergoing this treatment.

Momumental Physical Therapy & Performance is primarily a cash-based practice, allowing me to provide longer, one-on-one appointments and highly personalized care without the restrictions often imposed by insurance companies. Payment is collected at the time of your visit.
I am proudly in-network with Medicare and a TRICARE-certified non-network provider, which means I can treat beneficiaries with TRICARE Select and TRICARE Reserve Select plans.
For all other insurance plans, I am considered an out-of-network provider. All of my physical therapy services are eligible for reimbursement through your out-of-network benefits, and I will provide you with a detailed receipt (called a superbill) that you can submit to your insurance company for reimbursement based on your plan’s coverage. You may also use your HSA or FSA to pay for services.

For physical therapy clients, I offer in-office sessions in Cumming, or postpartum home visits on a case-by-case basis. For personal training clients — sessions can take place in my private studio or in your home, based on your location and my availability.

Not at all! While I have advanced training in pregnancy, postpartum, and pelvic floor rehabilitation, I work with women at every stage of life. Whether you’re dealing with pelvic floor symptoms, recovering from an injury or surgery, managing orthopedic pain, returning to exercise, or working toward a fitness goal, I’m here to help you move better, feel stronger, and get back to doing what you love.

While I don’t create individualized meal plans or meal prep for clients, I do provide general nutrition education and practical strategies to help support your health and fitness goals. We’ll discuss topics such as building balanced meals, fueling your workouts, hydration, protein intake, and creating sustainable habits that fit your lifestyle. If you have more specialized nutrition needs, I’m happy to refer you to a registered dietitian.
