Welcome to Momumental Physical Therapy & Performance (“Company,” “we,” “our,” or “us”). By booking services, accessing our website, or otherwise engaging with us, you agree to the following Terms of Service. Please read them carefully before using our services.
1. Scope of Services
Momumental Physical Therapy & Performance provides:
Mobile and in-home physical therapy for women’s health, pelvic floor rehabilitation, postpartum recovery, and related conditions.
Personal training and performance coaching for moms, female athletes, and individuals returning to activity.
All physical therapy services are provided by a licensed Doctor of Physical Therapy (DPT). Our services are designed to support recovery, education, and performance but do not replace emergency medical care.
2. Eligibility
You must be:
At least 18 years old to book services, or have a parent/guardian’s consent if under 18.
Physically able to participate in therapy and/or training as determined by our provider.
Willing to complete all required intake forms and disclose relevant health history.
We reserve the right to refuse or discontinue services if we determine that care would be unsafe or outside our scope of practice.
3. Payments
Payment is due in full at the time of service unless otherwise agreed upon in writing.
We are a cash-pay practice and do not bill insurance directly.
We can provide superbills upon request for you to submit to your insurance company.
Accepted forms of payment include [list payment methods: cash, major credit/debit cards, HSA/FSA cards, etc.].
Failure to provide payment may result in cancellation of future appointments.
4. Cancellations & Rescheduling
Appointments must be cancelled or rescheduled at least 24 hours in advance.
Late cancellations or no-shows will be charged 100% of the session fee.
Emergencies will be reviewed on a case-by-case basis at our discretion.
5. Health & Safety
You agree to provide accurate and complete health information before and during your care.
You understand that physical therapy and exercise carry inherent risks of injury.
By participating, you acknowledge these risks and agree to follow all safety instructions.
If you experience pain, dizziness, or discomfort during a session, you must inform your provider immediately.
6. Fitness & Results Disclaimer
Results from physical therapy, exercise, and training vary from person to person and depend on many factors including individual health status, consistency, effort, and adherence to recommendations.
We do not guarantee specific outcomes, performance gains, or timelines for recovery.
7. No Medical Emergencies
Our services are not intended for emergencies.
For urgent or emergency medical needs, call 911 or visit your nearest emergency department immediately.
8. Privacy & Confidentiality
We respect your privacy. All protected health information will be handled in compliance with HIPAA regulations.

9. Intellectual Property
All website content, materials, and educational resources are the property of Momumental Physical Therapy & Performance and may not be copied, distributed, or used without our written permission.
10. Limitation of Liability
To the fullest extent permitted by law, Momumental Physical Therapy & Performance is not liable for:
Any injuries or damages arising from misuse of exercises or failure to follow professional guidance.
Delays, cancellations, or interruptions caused by circumstances beyond our control.
11. Governing Law
These Terms are governed by the laws of the State of Georgia, without regard to conflict-of-law principles.
12. Changes to Terms
We may update these Terms of Service from time to time. Any changes will be posted on our website with an updated Effective Date.
